Thyroxine (T4)

Target Introduction

Thyroxine (T4) is a hormone produced by the thyroid gland. It is the main hormone secreted by the thyroid and plays a crucial role in regulating the body's metabolism, affecting nearly every organ and controlling important functions like breathing, heart rate, weight, digestion, mood, and growth (in children).

T4 exists in two forms in the blood:

  • Free T4: The active form of the hormone that enters body tissues.
  • Bound T4: T4 attached to proteins, acting as a backup supply in the bloodstream.

Relationship with Related Diseases

Measuring T4 levels is important for diagnosing and monitoring thyroid diseases, including:

  • Hypothyroidism (Underactive Thyroid): The thyroid gland doesn't produce enough thyroid hormone.
  • Hyperthyroidism (Overactive Thyroid): The thyroid gland produces too much thyroid hormone.
  • Other Thyroid Conditions: Thyroid nodules or goiter, which can sometimes cause high T4 levels.
  • Pituitary Gland Disorders: Abnormal T4 levels can indicate pituitary problems affecting TSH production.
  • Hypothalamus Disorders: Problems in the hypothalamus, which controls the pituitary gland, can affect T4 levels.
  • Congenital Hypothyroidism: Hypothyroidism present at birth.

Application

A T4 test is used to:

  • Diagnose hypothyroidism and hyperthyroidism.
  • Investigate other thyroid conditions, pituitary gland disorders, and hypothalamus disorders.
  • Screen newborns for congenital hypothyroidism.
  • Monitor T4 levels in patients undergoing thyroid hormone replacement therapy.

A T4 test is usually performed in conjunction with a TSH (Thyroid Stimulating Hormone) blood test for a comprehensive assessment of thyroid function. T4 levels can be measured with either a free T4 test or a total T4 test. Medical experts believe that free T4 tests are more accurate than total T4 tests, so they are used more often. A T4 test may be done as part of a thyroid panel.
