Biological Background: IL-6R/CD126 Function and Localization

  • Interleukin-6 receptor subunit alpha (IL-6R, CD126/gp80) is a transmembrane glycoprotein that exists as a membrane-bound form and a soluble isoform (sIL6R) generated by alternative splicing and proteolytic shedding.

  • Membrane-bound IL-6R binds interleukin-6 (IL-6) with low affinity; signal transduction requires association with the gp130 co-receptor to activate JAK/STAT, MAPK, and other pathways, regulating immune responses, acute-phase reactions, and hematopoiesis. Classic signaling via membrane IL-6R is predominantly regenerative and anti-inflammatory, with expression restricted mainly to hepatocytes and certain leukocyte subsets.

  • Soluble IL-6R (sIL6R) acts as an agonist by forming an IL-6:sIL6R complex that binds to gp130 on cells lacking membrane IL-6R, enabling IL-6 trans-signaling.
